Extended Aged Care at Home Dementia

  

What is an EACH-D package?

  

EACH-D packages are specific packages of care in the home which are tailored to the individual with complex care needs of care recipients with dementia. The service is tailored to meet the individual needs of people with dementia from all backgrounds including CALD and their carers who are eligible for the service. We endeavor to match the religious, cultural and linguistic needs, as well as gender of each care recipient (where possible). The workers are qualified bilingual and bicultural support workers and are provided with ongoing education and training in Aged Care.  They are given specific training in dementia to enable them to respond promptly in a sensitive manner to the needs of the care recipients, their carers, and families.

  

One OR more types of services may be provided by our Bilingual Personal Care Workers:

  

  • Personal Care
  • Domestic Assistance
  • Meal Preparation  
  • Social Support
  • Temporary in home respite care
  • Centre-based respite care
  • General – Administration & Care planning; and highly skilled dementia management
  • Specialized clinical services – clinical care by registered nurse & access/referral to other health and related services
  • Care services – activities of daily living (personal care, communication, household tasks, shopping and transport); nutrition, hydration and meal preparation; management of skin condition; continence management; support of care recipients with cognitive impairment; and aids for mobility and alertness.
  • Short term interventions to manage challenging behaviours and develop coping strategies
  • Short-term community respite care in response to emergency/ unplanned events

  

Who is eligible?

  

Any frail elderly person who:

  

  • Has an ACAT approval for EACH Dementia, or 
  • Is assessed by the Service Provider as requiring the type of dementia care able to be provided, and  
  • Lives or chooses to live in the catchment area of the program – Brisbane South

  

Fees and Charges

  

According to Commonwealth guidelines, you may be asked to contribute up to 17.5% of your pension towards your package. This fee will be levied and assessed by the Coordinator.
