Home and Community Care (HACC)

  

What is the HACC program?

  

HACC is a planned and coordinated care package of services tailored to the individual needs of frail and elderly people and people with disabilities to enable them to live independently in their own homes and to participate in their communities.

  

Who can use these services?

  

  • a frail older person  
  • a younger person with a disability  
  • a carer

  

How can IWAQ assist?

  

IWAQ is able to provide personalised care packages for Muslim and multicultural frail and elderly people and people with disabilities. This means we can develop personal care packages taking into consideration the special religious, cultural and linguistic needs of Muslim and multicultural people. We will endeavour to match their personal needs with a bilingual personal care worker (where possible).

  

One OR more types of services may be provided by our Bilingual Home Care Employees:

  

  • Personal Care
  • Domestic Assistance
  • Meal Preparation
  • Social Support
  • Temporary in home respite care
  • Centre-based respite care
